How Do We Refer Patients to The Diabetes Group Program?

1. Referral Instruction Sheet (Download Sheet)

Contains instructions regarding how to refer patient to the program

2. GP Referral Form (Download Form)

   The GP’s fill in section A and then give to the patient who brings to their 1st dietitian appointment

3. Patient Flyers (Download Flyer)

   To give to patients with Type 2 Diabetes and to keep in you waiting rooms

4. Patient Group Poster (Download Poster)

   To put in your waiting rooms to make your patients aware of the groups

Are these Diabetes Group Sessions instead of the 5 Allied Health Care Appointments?

No.  These services are in addition to the 5 individual allied health services each calendar year.
